This page is reserved for patients who have talked with our office and have been instructed to download a patient registration packet. If you would like an appointment to see Dr. Lyles or Dr. Brabec, please call our office for the log-in password to download the registration packet.
Patients are seen in a timely manner. Appointments are readily available. After you have completed the new patient registration packet which includes a health history form and after receiving pertinent medical records from your physician(s) who has treated or evaluated you previously, one of our staff members will call you to schedule a consultation and exam for you and your spouse. These appointments are usually made within a 1-3 week period after receiving the above information. If you have any questions, do not hesitate to contact our office. Our staff members are available to help you.
